Please complete the Card Access/Key Room Request Form (ARF/F004) and submit to ARF Coordinator/AVO at least one (1) week before access. The form can also be submitted via email to vet.arf@upm.edu.my.

Please complete the Animal Request Form (ARF/F001) and submit the completed form together with the copy of IACUC approval letter to ARF Coordinator/AVO. The documents can also be submitted via email to vet.arf@upm.edu.my.

Application must be made at least three (3) months before expected commencement of study.
